Tears and desperate pleas of relatives resonate in front of emergency departments—all for a single ICU bed. Innocent children afflicted with dengue and measles writhe in pain on thin sheets spread across hospital floors and corridors. This is not the scene of a single hospital; from the capital, Dhaka, to district and sub-district public hospitals across the country, the situation is equally heartbreaking. The healthcare sector of Bangladesh faces unprecedented strain under the dual attack of mosquito-borne dengue and deadly measles. Health department officials and medical experts fear this outbreak could spiral into a massive humanitarian crisis within the next month.
Severity of the Current Situation at a Glance
In the last 24 hours, 2 deaths were reported due to dengue fever, with 790 new hospital admissions. Meanwhile, the measles outbreak has claimed 3 lives and led to 1,111 new hospitalizations. Constantly rising crowds of infected patients are overwhelming emergency departments in almost all government hospitals in the capital, including Dhaka Medical College Hospital, Shaheed Suhrawardy Hospital, Sir Salimullah Medical College (Mitford) Hospital, and the Bangladesh Shishu (Children's) Hospital.
Beyond Borders: Aedes Rampage Spreads to Villages
While dengue was previously considered a disease centered primarily around the capital city, Dhaka, the situation this time is entirely different. The breeding and spread of Aedes mosquitoes have now swept across rural areas and villages. According to meteorological forecasts, excessive rainfall is expected in the coming days. Since Aedes mosquitoes breed in stagnant clean water inside homes and nearby containers like coconut shells or plastic vessels, there is a severe threat of infection rates surging even higher.
Prof. Dr. Md. Golam Sarwar, Head of Entomology at the National Institute of Preventive and Social Medicine (NIPSOM), stated, "Due to the lack of timely Aedes mosquito eradication and sustainable management activities, Aedes has spread from the capital to the village level. If city-corporation-centered breeding sources had been eliminated earlier, the spread to rural towns could have been prevented. Strengthening coordinated mosquito eradication at the local Union Parishad, Upazila, and Pourashava levels is now urgently required."
Severe ICU Shortages and Mounting Strain on Dhaka
Hospitals are suffering from an acute shortage of Intensive Care Unit (ICU) beds alongside general ward beds. Top officials from the Directorate General of Health Services (DGHS) reported that for individuals who have previously been infected with dengue multiple times, the severity of health complications and the risk of mortality are significantly higher this time.
District and upazila hospitals outside Dhaka are unable to accommodate patients due to excessive overcrowding. Consequently, critical patients are rushing to Dhaka in hopes of receiving urgent advanced care. However, Prof. Dr. Zahid Raihan, Acting Director General of the DGHS, stated that while the measles situation is coming under control, the dengue outbreak has exerted severe pressure on medical services. He added that 90% of patients infected with dengue for the first time recover quickly with basic medical care.
Expert Advice and Essential Initial Steps
Since medical management for both dengue and measles is largely symptom-based and similar, medical experts urge the public not to panic needlessly and to take appropriate steps in the early stages:
• Symptoms and Testing: If fever, severe headache, body aches, or loss of appetite occur, an NS1 antigen test for dengue should be conducted immediately.
• Special Precautions: Extra care and vigilance are required right from the beginning for children, pregnant mothers, the elderly, and individuals with comorbidities (such as diabetes, kidney disease, liver disease, or cancer).
• Blood Pressure Monitoring: Pediatrician Prof. Dr. Abid Hossain Molla noted that regular blood pressure monitoring is vital in dengue patients. Unless blood pressure drops dangerously low, there is no need to rush for ICU beds; primary management can be conducted in hospital outpatient departments.
• Dengue Corners: Medicine specialist Prof. Dr. A B M Abdullah advised that separate "Dengue Corners" should be established in all private hospitals across the country alongside government hospitals.
Call for Social Resistance and Coordinated Action
Recognizing the severity of the situation, directives have been issued to overhaul the healthcare infrastructure and execute comprehensive mosquito eradication drives. However, this epidemic cannot be contained solely through governmental or municipal initiatives alone. Citizens must step forward to keep their homes and surrounding premises clean and eliminate stagnant water. Only by building a widespread social movement across neighborhoods can the country be saved from this impending humanitarian catastrophe.
